Managing Safety Between Sessions

Sometimes life gets intense. This plan sets out what we’ll do — and what we’ve agreed to do — if things ever feel unsafe.

This plan is a practical reference for identifying risks and outlining steps to stay safe between sessions.

It can be used as part of your therapy work or as a personal record. It is not a diagnostic or emergency form.

Step-by-step plan of what to do immediately if I feel suicidal or at risk of harm
Short-term plans for keeping safe (e.g. keeping emergency contacts with me, devising a coping card with self-prompts, removing tablets, alcohol etc.)
How to involve other people (e.g. telling close family and friends so they can check-in with me between sessions, etc.)
Action to be taken by clinician if I do not attend an appointment and fail to make contact (e.g. telephoning me at home, informing GP or other services, contacting a friend or relative)
Names and telephone numbers of friends or relatives who may be contacted
