Sometimes things happen as part of our lives, and we need to stop and pause and look at the potential impact.

This page outlines the questions I may use when exploring risk in therapy.

It’s here for transparency and reference — you don’t need to complete it.

The form helps track current or historical risk and identify when extra support might be required.

Do you have any self-harm thoughts? If yes, what are these thoughts?
Do you act on your self-harm thoughts?
Do you have a history of self-harm? If yes, what method was used and the date of self harm.
Was medical attention necessary?
Have you recently suffered significant loss or threat of loss (including perceived loss of status or role)?
Have you recently disengaged with care or stopped medication?
Protective factors (e.g. family, support network, ability to seek help)
