Public Health Questionnaire No. 9 (PHQ9)

Depression

Depression can affect us all. This short form helps us take a snapshot of how it’s showing up for you right now.

This short PHQ form helps us take a snapshot of how depression may be showing up in your life right now. It isn’t a test, and there are no right or wrong answers — it’s simply a way for us to notice patterns and guide our work together. You can complete it before our session, or we can go through it together.

If you’re not sure about a question, just choose the closest answer that feels right for you.

What matters most is how you experience things, not the numbers on the page.
